One hundred and nine patients with hepatic malignant tumor underwent hepatic operations which included sixty-nine hepatic resections during 11 years from 1980 to 1990 at Department of Surgery, Catholic University Medical College.
Male was predominant(83.5%) to female and most common age were 6th, 5th decade.
96 patients(90.6%) were associated with chronic liver disease and Hbs antigen was detected in 20 patients(32.8%) among 61 patients examined.
According to the time, the incidence of small tumor(within 5 cm in diameter) was increased and the rate of diagnosis by screen test was also increased.
The positive of alpha fetoprotein(AFP) was 57.6% and it was neither influenced the detection of early diagnosis nor the operative methods.
The resection of tumor was undertaken the sixty nine patient(63.3% in resectability) which consisted of major resection in 31 patients(44.9%) and minor resection in 38 patients(55.1%).
The operative mortality(death within one month) was 26 patients(23.9%) among 109 patient but in the resection patients, the mortality were the 9 patient(13.0%) of 69 patients and the cause of death were hepatic failure in most common,
postoperative
bleeding. varix bleeding and sepsis.
The cause of death in long term survival patient were the recurrence of tumor in 58.1%, he patio faiure in 35.5% and varix bleeding and the site of recurrence were remnant liver in most common, lung and bone respectively.
The survival rate of resection patients were 53.8%, 39.8%, 36.5%, 1, 2, 5 years respectively and the factors of positive correlation with survival rate were curative vs palliative resection, cirrhotic liver or not, Child classification, rupture
of
tumor
but Hbs antigen, level of AFP, postoperative adjuvant treatment were not related to survival rate.
